Perineal Care
For all clients there are certain assessment points to remember:
- Assess for the any irritation, excoriation, inflammation or swelling
- Assess for any excessive discharge
- Assess for any odor
- Assess for any pain or discomfort
- Ask patient if there is any urinary or fecal incontinence
- Notice if the patient has a catheter in place
- If the patient has had recent perineal or rectal surgery
- Determine their perineal-genital hygiene practices
- When cleansing this area remember to clean from front to back. Another way to remember is to wash or wipe from “clean to dirty”
